[Music] hello my name is Jade I'm one of the doctors can I just check your name and date of birth please yeah and it's it's Jan Jan Wilkinson and um it's the 3rd of May 85. lovely thank you and what brings you in today oh I've just been really struggling with anxiety okay what do you mean by that what do you feel [Music] um well it makes my heart race um and I can um I can also feel quite sweaty okay and it's just I feel like that when just when I think really bad things
that can happen and get anxious yeah okay and how long has this been going on for a few months now and is there anything in particular you think might have triggered it yeah I know exactly what it was um so uh a few months ago um Layla my partner she was um so she was in a car accident gosh I know I know um I mean I I know at the time she you know she wasn't badly hurt but I just kept thinking about what could happen and um and just ever since then I I
just think everything bad it's going to happen all the time yeah that sounds really stressful is there anything in particular that you worry about yeah I am I I just really worry about my family and and their safety and are there any kind of situations that cause you to be more worried yeah I am so whenever um you know Layla goes out to drive I I am convinced she's gonna have an accident you know I've um I just worry about my children you know when they go to school I just thinking things are going to
happen to them and yeah and when you get these worries is there anything that you can do to help yourself calm down or get relief for them um yeah I mean it if if I maybe you know if I'm watching something on the television and I don't like one of the soaps or something um anything that can distract me that that can sometimes help yeah a little bit and have you ever had a panic attack at all yeah I am I I did have some panic attacks as a teenager um but I am I mean
I I hadn't had any you know for years I I did have one however about a week ago um oh I just got the school have been in touch because I've been texting the kids and then I just got into this state about I don't know they were going to get Social Services and take the kids away and I no I just I just couldn't catch my breath um she was great she um she managed to okay you know calm me down but um sorry I'm done I think I'm just going to be crazy no
no not at all it sounds like you've had a lot on your plate recently so I can understand why you know you're feeling really stressed about these things what would you say how has it been impacting your life full of these anxieties Majestic I think it's really affecting my relationships with my children okay yeah how's it been doing that God is all this constant checking how they are where they are they I think they just find it embarrassing and okay I I mean Layla's lovely but I I think even she now is just thinking it's
just all getting a bit out of hand okay and with everything that's been going on have you still been able to look after yourself oh yeah you know I I I'm eating drinking um yeah but it's just all consuming it's time yes have you been sleeping oh crikey no I just can't sleep you know just worrying about worse things that can happen those are pretty exhausting with all of this going on recently how's your mood been um yeah I I have felt a little bit low but that's just because I'm just getting fed up of
okay just constantly worrying yeah and do you still able to enjoy the things that you normally would [Music] probably because I just find these worries are all consuming so no things ever felt so bad that you've thought of harming yourself oh absolutely no I would never do that I mean goodness no you know I would never put Layla through that or my children no no I no I wouldn't no definitely not no and have you ever had any thoughts of wanting to harm others oh absolutely not no no not at all no no no no
okay okay sometimes when people are dealing with things like this they might turn to drugs or alcohol to help them cope is that something no no no I don't know I don't really like drink or anything like that no no okay so just a couple more questions about some things that you may or may not have experienced and do you ever get intrusive thoughts of like unpleasant things that you're not able to get out of your head um not really but but I have to say you know um if I see a car on the
street on the television I can't stop thinking kids are going to get knocked over we're going to be in an accident Layla's going to be in another accident I find it Sparks off me thinking that okay so it triggers some of your anxieties and yeah that it wouldn't necessarily just kind of pop into your mind out of no no no no no no and are there ever any kind of routines or rituals that you feel you need to do to help you manage these worries um no no no no no no to prevent kind of
bad things from happening no no I I do worry about bad things happening there and do you ever have any worries that someone's um trying to hurt you or out to get you no no no no no not at all no okay and do you ever feel like someone can or something might be able to hear your thoughts or interfere with them at all no no okay and have you ever been worried that someone might be able to control your thoughts or your actions no no okay do you ever see or hear things that other
people can't no oh what do your family and friends think about everything that's been going on God yeah I mean my children they're just embarrassed you know I have this constant texting of mine checking they're all right and I mean you know Layla's she's lovely and supportive but I just think it is beginning to weigh her down you know I'm constantly asking her for reassurance that everything's gonna be all right yeah do you think these worries that you've been having are reasonable no I am I I do know that they are getting out of hand
yeah okay and is there anything in particular you were hoping would be able to do to help you um I was hoping um just hoping I don't know maybe maybe some counseling or someone I can just talk to just to to make me feel normal again really well thank you so much for telling me all of that I think it'd be best if I speak to my team about what we can do to help you and I can give you a call a bit later on if that would be okay yeah thank you have you
got any other questions for now no thanks for your help though okay thank you very much today I assessed Jan a 38 year old lady presenting with generalized anxiety which is affecting her relationships she has a history of panic attacks and describes multiple social stresses on examination Jan appeared anxious and Restless she demonstrated good eye contact and it was easy to build a report there were no abnormalities of speech she reported feeling generally on edge with difficulty relaxing at times her mood has been a little low Jan reports ruminating anxious Thoughts with evidence of catastrophizing
