I always say that patients with some type of cardiovascular disease should not use anti-inflammatory drugs. Because the anti-inflammatory can increase the risk of other cardiovascular diseases, such as a stroke, a heart attack, and so on. But what can a person with cardiovascular disease use when they have pain?
Or when you have some kind of inflammation? An alternative to anti-inflammatory drugs is the use of dipyrone, which is also one of the best selling drugs in the world. Dipyrone has analgesic, antipyretic and anti-inflammatory properties.
That is, it can be used for various types of pain: headache, joint pain, muscle pain. In addition, it also lowers fever and also acts as an anti-inflammatory. It has the ability to inhibit the production of prostaglandins, which are substances involved in inflammation and pain.
So, it acts by inhibiting COX, which is the cyclooxygenase enzyme, responsible for the production of prostaglandins. These prostaglandins are chemical mediators that have several functions in our body. They regulate body temperature, vasodilate and sensitize pain receptors, when we are injured or when there is some inflammation in our body, cyclooxygenase is activated and begins to produce prostaglandins.
This ends up increasing our pain, increasing inflammation, sometimes even causing a fever. Dipyrone then acts by inhibiting COX, reducing prostaglandins, then you have a reduction in pain, inflammation, fever, although it has an anti-inflammatory action, this action is not as potent as that of non-steroidal anti-inflammatory drugs, that you know around, diclofenac, nimesulide, ibuprofen. Although its anti-inflammatory action is not as potent, it is preferable mainly in people with some cardiovascular disease, for pain relief.
As I said before, we Cardiologists prescribe dipyrone for pain even in patients admitted to the ICU, after cardiac catheterization, after a heart attack, or even in patients who have just had heart surgery, so it is a very safe medication. We know that using anti-inflammatory drugs can increase the risk of cardiovascular events in patients with a history of cardiovascular disease. So, it increases the chances of having a heart attack, stroke, heart failure, in addition to anti-inflammatories inhibiting COX, inhibiting prostaglandins, the inhibition of cyclooxygenase can also affect the production of other substances such as prostacyclins, which have vasodilator properties, anti-platelet aggregation, that is, help to prevent the formation of clots blood.
So in patients with cardiovascular disease, anti-inflammatories can reduce the production of prostacyclins, increase the production of thromboxanes, which are pro-thrombotic substances. And what does that entail? Increased cardiovascular risk.
Heart attack, stroke and heart failure. That's why in patients with a history of heart attack or other heart diseases, the patient has to avoid the use of anti-inflammatory drugs. Instead, we recommend the use of analgesics such as dipyrone, in appropriate doses, of course, for short periods and always under medical supervision.
It's important for you to understand that it's not just going to the pharmacy and buying dipyrone, you have to get guidance from your doctor because this recommendation depends on your case, your history of illnesses. It is important to know that paracetamol can also interact with other medications, so it depends on the health condition of each patient. For example, paracetamol should be used with caution in patients with liver disease, or in combination with other drugs that affect the liver.
Because excess paracetamol can lead to liver damage. In addition, there is a maximum dose per day. Beyond that dose, it can become toxic and in some cases even fatal.
