welcome to another episode of I have the balls to talk about it and today we're gonna talk about testosterone and this is a pretty pretty often talked talked topic we see we hear it and see it misunderstood topic misunderstood as well because there's so many people who are out there making these videos talking about testosterone and foods and this and that today we're gonna look at it from medical perspective how to boost testosterone and a lot of men I'm 4250 a lot of many we worry about it and like a certain age we're like Oh
am I too low a might too high and I even came to see you with my testosterone levels say Andrew can you check this out so we're gonna try to you know break it down make it as simple as possible for you to guys learn and understand from a medical perspective about testosterone so okay so number number one I just want to say testosterone supplementation it's a drug it's a medication so this is I don't advocate anybody using it for when it's not medically necessary you know to boost yourself for training or for extra sex
life what yeah that's what I wanted before all right and you know I need to train and when you looked at them you said they were yeah so there is it's a drug with side effects it's a hormone and you know I'm you know advocating for the the proper use under proper supervision in the proper selection of patients which we're gonna talk about so number one what are the symptoms of someone who is low in testosterone okay so I would say the Communist cause that people come to see me use based on where I test
for testosterone levels are low libido low sex drive what what's a low sex drive well because there is there a measure to that it there's no measure there are actually there are questionnaires the ad am adam questionnaire it's to check for androgen deficiency in the aging male and it's a series of questionnaires rating how your sex drive is and it gives you score but these are just sort of tools that we use most patients come in and say look you know I'm not in the mood Moe my wife thinks I'm cheating on her or something
I'm just not in there I'm just I don't have the same sexual desire that I had five years ago that really comes down to your testosterone often often okay yeah we'll talk about that we'll get into some of the details but that's the commonest cause a low libido erectile dysfunction alone you know like III feel like I'm in the mood but my erections aren't aren't as strong as they were that could be a sign of low testosterone some people say well my thought process is more cloudy fatigued I'm trying to do exercise and I'm barely
making it through and I'm not building any muscle mass and I'm gaining some weight - no matter how hard I try I can't I don't see any results so these are some of the general signs of testosterone deficiency this is something that can be easily measured it's a simple blood test you know I see patients who are on testosterone from other doctors or they picked it up at the gym from on the black market and they've never been tested I think that's unsafe it's very easily we can check the testosterone levels it's a blood test
what I should be done and it's a blood test it should be done in the morning because your testosterone actually varies throughout the day so to get the the levels at a standard level it's normally done in the morning let's say between 8 and 10 o'clock we like to see two tests maybe a couple of weeks apart or a month apart to really confirm that the testosterone is low and and there's different forms of testosterone it's it's a I think we can say that for another day but there's there's the total testosterone and there's the
bioavailable and free testosterone all that to say is the most accurate is what we call the bioavailable testosterone I really insist on all my patients getting that because I've had patients who have been symptomatic and haven't been treated because their total testosterone is low it's sorry is normal and then we actually dig down deep to the more sensitive bioavailable it's actually low okay okay that's number one and I don't need the testosterone to be you know zero if someone is even in the lower range of normal but symptomatic today's guidelines say you know treat them
give them a shot of three months and then see how they go you don't have to be in the basement and you also have to take into account the age because a testosterone level of a 20 year old she's gonna be much higher than a 60 what age doesn't decline like it starts to decline in your 30s by a couple of percentage a year on average is it like compounded or like he's 2 and then he'll either to another well yeah I mean he you know what keeps dropping in some men but some minute drops
higher than another men or faster than other men and is it a genetic factor I don't think there's any genetic factor I've come across in terms of testosterone deficiency there are other medical reasons I'm just talking about your standard case of low testosterone there are actually some brain tumors that can cause an increase in the hormone prolactin which can decrease your whole you know whole axis of brain to testicles cause actually the testosterone production is controlled by the brain hormones that come from the pituitary gland Wow so uh you know just to keep it simple
I'm talking about the run-of-the-mill low testosterone levels and so one of the common questions once we diagnosis how do I treat it there are although you see a hundred ads and claims there are no proven testosterone boosters in terms of that's amazing because the market is on YouTube are like it's just everybody and it's like it's it's become like yeah industry and I just actually searched last week didn't it did an online search to see if there's any studies proving testosterone boosters helped and I came across no studies that actually showed have a patient who's
starting and before he said before he wanted to try you know medical testosterone let's say he wanted to try testosterone booster I said okay look let's do a blood test now he's gonna take it he's good you're gonna take it for a month and then he's gonna come back in a month to do another blood test so it's only a sample size of one patient but a little internal testing so testosterone is normally given in the form of testosterone and it can be given in a cream that you put on your skin every day there's
a so you put a cream this is somewhere specific well you actually have to switch places in your body because if you put it in the same place every day the muscle in that area is actually gonna grow it's gonna and you're like if I put cream here my those muscles are gonna grow yeah yeah so it works it works but if you - if you want to grow your whole body you're gonna kill yourself with testosterone overdose so that's why it's it's meant to be placed in certain areas you have to be careful with
this cream because if you don't wash your hands and you touch your your your kid or your girlfriend they can absorb some of that testosterone and produce some male characteristics your girlfriend will grow a beard so there is a use of testosterone in women but again we'll save that for another time so there's some nose creams and underarm gels I find they're all easy to use and actually there's an injection that you give in the butt every two weeks that often gives the biggest bang for the buck they all work and they all have benefits
and and inside effects that can be do you can have from them and I normally discuss a different option with the patient and see which one they want to start with and then we follow up after a month or so to see if they like it but there are things that have to be followed so you have to and most of them are more historical for example there was a fear of heart disease coming from testosterone now that that launched a big scare about each years ago although it was based on completely false information seized
yeah they giving testosterone boosting of testosterone can cause heart attacks so this was actually based on a horrible medical study with with improper data problem is it made it to CNN and the New York Times testosterone bad for the heart and then when this scientist got hold of it and tore apart the study the damage was done so for years people were not being treated based on these false data actually the data may be the opposite so the a couple of things that we know that one men with low testosterone not treated but just low
testosterone are actually at increased risk of having heart disease and then men who are treated with testosterone some of the things that can cause heart disease actually improve some may be your cholesterol levels or your diabaté diabetes some of these things may actually improve so they're the opposite of what we thought before and I don't want to go on record as saying that testosterone is good for your heart but that may be so that giving testosterone to someone who is deficient again I'm not talking about someone just wants to bulk up and their doctoral levels
are normal there may be some protective this is being studied now and we'll probably know more about that in the future the other thing is we were worried about this prostate will it cause prostate cancer that was a big fear but there is actually no significant data to link testosterone replacement and prostate cancer at this point so let's just call it a wash you know something we check we checked the prostate in the follow-up but I don't think there's any strong heart data that gives a personal risk of having a prostate cancer in some cases
especially with the injection your blood levels your hemoglobin levels your red blood cells can increase with testosterone so if it increases above the normal you may be a little risk of having some some blood clots or heart disease in those rare cases that can be treated by switching the form of testosterone that you have or actually by giving by donating blood to bring it to keep your levels down to a normal level so one of the more important reasons why if you take testosterone do it properly and have it followed properly on a professional by
a professional yeah because it can be it can be a life changer in terms of how you feel you know tired lower low libido poor erection and now you can feel like a million bucks which is how a lot of my patients feel after taking so thank you so you got to do it problem they think when they take the shot or the cream they feel the difference there are some there are some patients who feel minimal difference but I have a lot of patients where like I said a life-changing experience and they they didn't
realize how bad they've been feeling for the last five years until they take it yeah so if you feel any of these symptoms get it checked out because you may be suffering now you asked me before about the test and the testosterone it's not the only thing I test when these patients come in because other things can mimic low testosterone thyroid disease depression and all in some of these other things and so and other diseases can cause your testosterone to decrease okay so you know it really don't just go to the gym and find some
guy you know a postural get it tested you may have low testosterone that may be needed to be treated just you know properly and followed properly but you may have another illness which is causing similar symptoms that may need to be treated in them I have a question when you take take testosterone like when you get a cream or whatever is something you do once twice or is it something you need if you have low this is right so this is I just something that bothersome patients they say well I take it for a month
and I'm okay no this is not like like you have a you know an infection you take antibiotics for a week and you're cured you have low testosterone you have to be on it long term Wow okay and and when people think about testosterone they freaked out but you know yes how many patients do we know out there who are on thyroid supplementation it's another hormone and they take it you know this is a normal hormone that your body produces and if it's low we can give it to you safely effectively and and make you
feel better but it's it's it's a long term it's a long term commitment Wow you know it's like blood pressure diabetes it's low testosterone is disease it needs to be treated long term there's one caveat for younger men it can't affect your fertility so if you if I have a I see rarely a 28 year old who hasn't had kids yet who has low testosterone and really symptomatic you can I either put them on testosterone and stop it every once in awhile we cycle them to get their testes to work again or there are actually
some medications which are used in fertility which are not you're not giving testosterone but you're giving a medication to boost the testicles to produce more testosterone so that usually spares the the the fertility although it doesn't give often the same bang for the buck that giving a full testosterone does but it can improve symptoms and spare fertility and in many men Wow sign so that's that's so interesting like I said and and you know being a trainer as well and it's like you see super bombarded with articles blogs videos how to boost testosterone this that
and it's it's so confusing especially for you being a doctor you must look at this and be like this is people are so misinformed in so many ways and we don't know what's real or not anymore so that that even makes it much harder so I guess they just they just do a blood test yep see their medical doctor yep check the testosterones I'm gonna say one more thing because senior medical doctor push it push the issue because there are so many doctors out there who are afraid to test it afraid to treat it because
of misinformation and I've had like I said patients who've had testosterone low testosterone for years and they have the blood tests in hand and the doctor said well I don't feel comfortable giving you testosterone push it seek another opinion come see me text me in the bottom will set you up with someone you know but but but push the issue if you feel tired fatigue low sex drive your erections aren't what they should be maybe cloudy thought and you don't feel yourself check it out check it out and and and listen to listen to yourself
