So, today I want to talk about seven main laboratory tests that we request in our practice, in our Clinical Cardiology routine that all need to be normal for the patient to have a better quality of life and for him to live longer.

So, I'm going to talk about the main tests that, if altered, can lead to heart disease, whether it's an increased risk of having a heart attack, having a stroke, having heart failure. So, you have to have all these exams up to date to reduce the probability of having a heart disease in the future.
But, here we go, without further ado! What is the number 1 exam that has to be normal? It's fasting blood sugar, folks.
So, the blood glucose shows whether your sugar is working correctly in your body, or if you already have pre-diabetes, which is when the glucose is between 100 and 125; or, if suddenly you're even diabetic, it's when your fasting blood glucose is greater than or equal to 126 mm/dl. So, the first exam and one of the most important, if not the most important, that you have to have well controlled is your glucose, folks! So, your cardiologist will always routinely ask for your fasting blood glucose along with your glycated haemoglobin!
I always say in my videos, glycated hemoglobin above 7, the patient is losing years of life; greater than or equal to 6. 5 is already considered diabetes. But above 7 is poorly controlled diabetes.
So, the first exam you have to have there, measured the fasting blood glucose, it is below 100, dosed together with the glycated hemoglobin and saw that it is below 5. 7 everything is ok, it means that your blood glucose is fine controlled and you don't have diabetes, ok! For those who don't know, I always say: diabetes is a very serious disease, it's a potentially malignant disease, I usually compare diabetes to an evolving cancer, because poorly controlled diabetes often ends the person, kills the person even more faster than cancer, for example.
So, if you have diabetes and, above all, poorly controlled diabetes, look for your cardiologist as soon as possible so that you can have the treatment, because today there are countless pharmacological and non-pharmacological treatments to improve your quality of life and make you live longer, even though you're a diabetic, yeah! But let's go! Second exam: it's HDL cholesterol, folks.
HDL cholesterol, for those who don't know, is considered the good cholesterol. In fact, we have cholesterol circulating between the liver and our heart. HDL and LDL are nothing more than the little trucks that will pick up cholesterol.
So HDL picks up cholesterol from the arteries in the heart and takes it back to the liver. So if you have high HDL levels above 50 mg/dl, in current guidelines they are tolerating above 40; until a while ago it was above 50 for women and above 40 for men, nowadays we are tolerating it, accepting levels above 40 as well-controlled levels. So if you have HDL cholesterol, which is the good cholesterol, below 40, you have to reverse this situation!
And how do you increase HDL cholesterol, which is the good cholesterol? It's doing a lot of physical exercise, at least 150 minutes of moderate exercise per week. I already recommend to my patients at least 300 minutes a week of aerobic physical activity and also weight training activity.
Mainly the elderly people go into sarcopenia, the muscle becomes more atrophied, at that time after the age of 60, it is even more important that you also do weight training, it will help you to become stronger as a whole and also protect your heart. And another thing is to consume good fats, extra virgin olive oil, avocado, chestnuts, nuts can also help to naturally increase HDL cholesterol, which is the good cholesterol. But, let's go to the third exam that can't be changed so that you live longer and live healthier?
After HDL, it's LDL cholesterol. LDL cholesterol is the one that will take cholesterol from the liver and throw it into your heart arteries. The higher the level of LDL cholesterol, the greater the probability of having a heart attack, of having a stroke because LDL cholesterol accelerates that process that I always talk about, accelerating the process of atherosclerosis, which is the accumulation of fat in our vessels, clogging our arteries.
So, if you have high LDL, you also have to control it. Unlike what I'm going to talk about later, triglycerides, cholesterol is more difficult to have strict control, because LDL cholesterol depends a lot on our genetic background. So, if we have a parent who has high LDL cholesterol, most likely we will too!
It is clear that a good diet, physical exercise, can reduce LDL cholesterol, but it will not reduce the amount that reduces triglycerides, for example. The LDL cholesterol goal will vary from patient to patient. For example, that patient who has already had a heart attack is at very high risk, so his target LDL will be below 50!
If the patient is at high risk we tolerate an LDL below 70, and so on! Depending on the risk classification, we have this classification and decide the reference LDL for each patient, which can vary from 130 for low risk patients to below 50, which is what I said, for patients with very high cardiovascular risk. So, another test that has to be normal is your LDL cholesterol.
All of this you will write down and then you will talk to your doctor to be able to normalize all these exams. So let's go! Are we talking about HDL cholesterol, LDL and what comes next on the lipidogram?
It's the triglycerides, folks! So, the fourth exam that has to be normal: the triglycerides. Triglycerides are fats that come from carbohydrates, so is excess rice, bread, pasta, potatoes, soda, juice, beer.
All this will increase triglycerides. That's why carbohydrates and excess make a person fat too, accumulate more triglycerides, accumulate more fat. So, with regard to triglycerides, there is not much of this issue of risk classification.
You have to keep the triglycerides always below 150. What are you going to do then? As I said, unlike cholesterol which lowers little with a change in lifestyle, I've seen several patients with triglycerides above 500, who with diet and physical exercise, improved diet, changing their mindset, taking care of their health, were able to naturally drop below 150.
Then sometimes it drops from 500 to 150 just by changing lifestyle! But values above 500 tend to start pharmacological treatment, with medication, right from the start! But then, it's keeping triglyceride levels well controlled, ok!
This will be very important too! So, we're talking about glucose, we're talking about HDL cholesterol, LDL cholesterol, we're talking about triglycerides, now let's talk about: uric acid, folks. So, uric acid as well, we know that the increase in uric acid is directly related to the acceleration of the atherosclerosis process.
Especially if you have low LDL, high triglycerides, uric acid above 7, what can happen? Elevated uric acid gives gout. Many people complain of gout, you start to have those gouty arthritis, especially with uric acid above 10, but that's not the main problem!
The main problem is that uric acid also inflames the arteries of the heart, accelerating the process of atherosclerosis. So, for women, uric acid above 6 is already high and for men above 7 it is already high! The same thing, to control uric acid is to exercise and avoid foods that increase uric acid; red meat, some seafood, everyday beer also increases uric acid, beans themselves slightly increase uric acid as well.
So, if you join a good Nutritionist, you will be able to manage and regulate it correctly! To recapitulate, then: glycemia has to be normal, LDL cholesterol, HDL cholesterol, triglycerides, uric acid. What is the sixth exam that has to be normal?
It's your TSH, folks! TSH shows whether your thyroid is working well or not. When TSH is too low it's because you're producing too much thyroid hormone, so you have hyperthyroidism.
If your TSH is high, the pituitary gland is producing too much TSH hormone, that means you have hypothyroidism, lack of production of thyroid hormones. So, we know that both hypothyroidism and poorly controlled hyperthyroidism decrease, in addition to greatly reducing the quality of life of the person, it can also lead to a decrease in survival. So, the TSH has to be normal, ok!
And, the same thing, as I say, that a diabetic. . .
A good cardiologist has to know how to take care of diabetes, a good cardiologist has to know how to take care of your thyroid too, okay! So, TSH levels also have to be up to date, they have to be normal too! And, seventh exam.
. . So, I'm talking here about the main exams that have to be normal so that you have a better quality of life and that you don't have cardiovascular diseases.
What is the seventh exam? It's the creatinine, folks. So, creatinine estimates the function of your kidneys.
So, the lower the creatinine, it means your kidney is working better! Levels above 1. 2 mean that your kidney may not be working 100%.
So, it is also very important that you routinely measure your creatinine to know your kidney function, to know if everything is ok. Why? Because kidney failure, that is, weak kidneys will make your heart weak, because a weak kidney increases blood pressure and we know that high blood pressure is the main risk factor for cardiovascular disease.
So, it's important that you always have your creatinine very well controlled. All right, guys! Look, there are seven exams then!
